A Platform for Community Healthcare

MEDTRAC Health is built as a healthcare infrastructure layer that can operate across clinics, community health centres, mobile and pop-up settings, and remote communities. It is designed for environments where connectivity, specialist access and healthcare infrastructure are limited — initially focused on African deployment, with applications for underserved communities worldwide.

Core Capabilities

MEDTRAC Health combines the components needed to deliver, manage and monitor care close to where people live:

  • Digital patient records — structured, portable health records that follow the patient across sites and providers.
  • Community screening — guided screening and assessment workflows for health drives, clinics and field programmes.
  • Preventive healthcare — risk awareness, reminders and programmes that support earlier intervention.
  • Digital questionnaires and assessments — standardised intake, triage and follow-up forms for consistent capture.
  • Clinical decision support — practitioner-facing tools that support, but do not replace, professional judgement.
  • Community clinics — a local healthcare access point supporting screening, consultation and referral.
  • Mobile and pop-up healthcare — deployment models for markets, temporary sites and outreach.
  • Remote consultations and telemedicine — access to remote practitioners and specialists.
  • Follow-up and monitoring — structured check-ins and ongoing care tracking.
  • Offline-first healthcare delivery — core workflows continue without always-on connectivity and synchronise when it returns.
  • Local edge AI — on-site assistance and triage support that works close to the patient.
  • Health education — accessible information for patients and community health workers.
  • Referral pathways — clear routing from community level to clinics, hospitals and specialists.
  • Anonymised health analytics for health authorities — population-level insight to support planning and response.

How It Works: From Assessment to Care

- Assess: Guided questionnaires and screening capture structured health information on phone, hub or clinic systems.

•Triage: Digital triage helps route people to the right next step — self-care, community support, clinic or remote review.

•Support: Practitioners and community health workers use decision-support tools, with the professional remaining in the loop.

•Connect: Telemedicine and referral pathways extend specialist and diaspora support into underserved settings.

Offline-First by Design

Essential local services can continue when internet connectivity is interrupted and synchronise when connectivity becomes available again. MEDTRAC Health is designed so that screening, records, triage and follow-up keep working in the field, with central systems updated when the link returns.

Evidence & Real Clinical Use

Concept and platform work began in 2020 around multi-signal clinical decision support. The platform has been refined through real patient workflows at the Atlantis Clinic in Malta, with over 1,000 people assessed through evolving digital health intake and screening workflows. Community and ministry-backed African pilot programmes are now being prepared.
